Are we accepting new patients?

Yes, we are accepting new patients as long as we are in network with your insurance. 

What services do we offer?

Our clinic provides comprehensive primary care services, as well physical and occupational therapy for our established patients. We are committed to providing personalized and compassionate care to help our patients achieve their health goals. 

What insurance do we accept?

We are currently accepting Partnership and Medi-CAL patients.

We are currently not accepting commercial insurance.

How do I become a patient?

We would be delighted to have you as a patient. Please ensure we are accepting your insurance then you can pick up (from our clinic) or download and print our new patient packet. Once completed, you can drop it off at our office (2404 Newburg Fortuna CA 95540) for processing. Thank you!

How soon can I get an appointment?

Currently, we are booking 2 - 3 weeks out.

How do I access the patient portal?
